New Patient Portal for QCH Patients

OTTAWA — October 24, 2026 — Queensway Carleton Hospital (QCH) has moved to new online patient portal called MyChart. The existing ConnectedCare Patient Portal will stay accessible until December 18, 2026. After the transition, patient health records starting from January 2023 will be accessible via MyChart.

Patients with an existing MyChart account from another participating hospital do not need to register again. Their Queensway Carleton Hospital information will automatically be accessible.

• Upcoming appointments and appointment reminders
• Laboratory and diagnostic imaging results
• Progress notes and clinical documentation
• After-visit and discharge summaries
