Consent for Treatment Information

You will be asked to sign a consent form for certain tests, procedures and treatments.

Information regarding the benefits, risks and options must be provided by the healthcare practitioner providing your care. This is called “informed consent.” You have the right to refuse treatment and it is important for you to understand the implications of your decision.

If you are unable to consent to treatment for any reason, you must appoint a Substitute Decision Maker. At Patient Registration, you will be required to give the name of your Substitute Decision Maker.

Information regarding Substitute Decision Makers and Power of Attorney kits is available through the Social Work Department located on Level 1.
