Successful SHIIP Pilot Improves Care Coordination

Ottawa, ON — Queensway Carleton Hospital (QCH) is highlighting the success of a recent pilot of a digital health solution that enables secure, real-time access to patient information across healthcare providers.
The pilot was initiated and funded by the Ottawa West Four Rivers (OWFR) Ontario Health Team (OHT), supporting two community partners - JW MacIntosh Community Support Services and Seniors’ Community Services - to evaluate its impact in real-world care settings.
As background, QCH hosts the Shared Health Integrated Information Portal (SHIIP) with oversight from Ontario Health East for health care providers in the Ontario East Region. SHIIP supports a seamless exchange of patient information between hospitals, primary care, and community providers, giving clinicians a more complete view of a patient’s health journey and enabling more coordinated, informed care at both the patient and population health management levels. In this pilot, SHIIP brought community services’ use to the Ottawa and Valley Area for the first time.
Findings from the pilot show improved care coordination, better visibility into patient interactions across the system, and stronger collaboration across health, community, and social service sectors.
“SHIIP allows providers to see the full picture of a patient’s care, which is critical for delivering timely and effective treatment,” said Lindsay Wyers, Vice President, Digital Transformation & Chief Information Officer at QCH. “This pilot reinforces the value of connected systems in improving both patient outcomes and provider experience.”
To support broader awareness and adoption, the OWFR OHT Digital Health, Privacy and Information Sharing Committee will be hosting a session on May 12 for additional community partners to learn more about SHIIP and its potential use cases.

I’ve found SHIIP to be a valuable and user-friendly tool for our team. Being able to see real-time updates on a client’s status—whether they’re in hospital or moving to long-term care—has made a big difference in how we coordinate and support transitions.
