In partnership with Regional Geriatric Program of Eastern Ontario (RGPEO), Queensway Carleton Hospital (QCH) offers Specialized (out-patient) Geriatric Services by our Geriatric Assessment Outreach Team (GAOT). This team specializes in caring for seniors in our community and provides in-home assessments to at-risk older adults who are 65 years or older, for whom leaving home is physically, mentally, or logistically difficult. Assessments focus on functional, psychological, cognitive, and physical health. Our Outreach Team is committed to working with the Primary Care Providers and community services agencies.

Older adults can be referred by their Primary Care Provider, other healthcare professionals, family, caregivers or themselves, and referrals are accepted for people living in the community.
